Eye & Ear Program of La Plata and San Juan Counties Colorado, Inc. (Through the Kiwanis Club) c/o Robert H Tyner, President Po Box 121 Durango, CO 81302-0121 Services and Eligibility The program offers sliding scale prices for hearing aids, up to a 70-80% discount. Residents of La Plata and San Juan counties in Colorado of all ages are eligible. Students who qualify for free school lunches or adults with incomes less than 145% of the National Poverty Level are eligible. To Apply People should contact the San Juan Basin Health Department and ask for help from the Eye and Ear Program. Michelle Romero will work with them and tell them what it takes to qualify.
